Keto for Athletes: Performance, Protein & Fueling
Keto changes fuel use more consistently than it improves sport performance. The 2024 International Society of Sports Nutrition position stand concludes that ketogenic diets have largely neutral or detrimental performance effects compared with diets higher in carbohydrate, despite increasing fat oxidation.1 An athlete should test the demands of the event, not assume that higher fat burning means faster performance.
This page addresses healthy exercising adults. Diabetes medicines, pregnancy, a history of disordered eating, kidney or liver disease, unexplained fatigue, menstrual disturbance, or an athlete still growing requires clinical and sports-nutrition guidance.
What changes after carbohydrate restriction?
Muscle and liver glycogen fall as carbohydrate availability drops, while fat oxidation and ketone production rise. Adaptation time, diet definition, testing method, training status, energy intake, and event intensity differ across studies. A result from ultramarathon runners cannot be assigned to sprinting, team sport, weightlifting, or a novice gym program.
The FASTER study by Jeff Volek and colleagues compared 20 highly trained ultra-endurance athletes who habitually ate long-term low-carbohydrate or high-carbohydrate diets. The low-carbohydrate group reached much higher fat-oxidation rates, while muscle glycogen use and restoration during a three-hour run were similar between groups.2 It was a cross-sectional comparison, not a randomized performance trial.
Louise Burke and colleagues randomized elite race walkers during an intensified training block. After three weeks, the ketogenic low-carbohydrate, high-fat group increased fat oxidation but had worse exercise economy and did not gain the race-performance benefit seen in the higher-carbohydrate groups.3 The finding has been reproduced in that research program.4

Targeted and cyclical keto are informal strategies that add carbohydrate around training or on selected days. Protocols vary, and direct trials do not establish one dose or timing rule. If adding carbohydrate restores training quality, the athlete is no longer testing strict keto; that can still be a rational performance choice.
Set protein from context
The adult DRI RDA remains 0.8 g/kg/day and was designed to meet the needs of about 97-98% of healthy adults.5 The 2025-2030 Dietary Guidelines give a general daily goal of 1.2-1.6 g/kg/day.6 Sports guidance from Shawn Arent, Douglas Kalman, Chad Kerksick, and colleagues commonly uses 1.4-2.0 g/kg/day for exercising adults, with context for training and energy availability.7
A meta-analysis led by Robert Morton found that gains in fat-free mass during resistance training averaged little additional benefit above about 1.62 g/kg/day in energy-sufficient study conditions.8 That estimate is not a universal ceiling, does not prescribe protein during a cut, and measures fat-free mass rather than muscle alone.

Protect energy availability and food quality
An athlete can meet a carb ceiling and still underfuel training. The 2023 IOC consensus on Relative Energy Deficiency in Sport describes health and performance consequences when energy intake remains too low relative to exercise demands.9 Watch for declining performance, persistent fatigue, poor recovery, recurrent injury, menstrual disruption, low libido, mood change, or unexplained weight loss.
Food quality matters independently of ketosis. Include varied protein foods, non-starchy vegetables, suitable fiber sources, and mostly unsaturated fats. A diet heavy in butter, processed meat, and low-fiber convenience products is not made adequate by a ketone reading.
Fluid and electrolyte replacement should reflect sweat loss, climate, session duration, diet, blood pressure, kidney and heart function, and medicines. A fixed sodium prescription for every athlete is unsafe. Use changes in body mass around training, thirst, urine, symptoms, and professional assessment rather than a generic packet count.
Does creatine work differently on keto?
Creatine monohydrate has strong general evidence for repeated high-intensity exercise and resistance training.10 Keto-specific evidence is sparse. One 2024 study included 19 recreationally active adults on a diet with less than 30% of energy from carbohydrate, which is not necessarily ketogenic, and tested interval performance rather than ketosis.11
Plain creatine contains no carbohydrate, but that does not prove ketone levels are unaffected in every setting. Run a controlled personal trial
Change the diet outside a competition peak when possible. Keep the training block, sleep, caffeine, testing conditions, and key performance tests as stable as practical. Record:
- pace, power, or load at a repeatable effort;
- total training volume and ability to complete planned intensity;
- recovery, sleep, mood, and gastrointestinal symptoms;
- body weight trends without treating initial water loss as fat loss; and
- menstrual, injury, and energy-availability warning signs.
