Acetoacetate
Acetoacetate is the first ketone body formed in liver mitochondria during ketogenesis. Urine nitroprusside strips detect it primarily, while blood meters usually measure D-beta-hydroxybutyrate (BHB). Acetoacetate can circulate to tissues, be reduced to D-BHB, or decarboxylate to acetone.
How is acetoacetate made?
Acetoacetate is formed from acetyl-CoA in liver mitochondria. Hepatic ketogenesis increases when fatty-acid delivery and oxidation rise, including during fasting and sustained carbohydrate restriction.1 The ketone-body guide follows the complete pathway.
What happens to acetoacetate after it's made?
After export from the liver, acetoacetate can be oxidized by tissues with mitochondria. It can also be reduced to D-BHB according to redox state or decarboxylate to acetone. These are not mutually exclusive fates, and their relative amounts change with physiology.
Why do acetoacetate readings change as you adapt to keto?
Ketone urine strips use a nitroprusside reaction that detects acetoacetate but not BHB.2 A urine result reflects ketones accumulated since the last void and varies with hydration, renal handling, and timing. During DKA treatment, urine acetoacetate can appear to rise while blood BHB falls. In dietary use, a fading strip cannot by itself prove adaptation or the absence of ketosis.
