Exogenous Ketones

Exogenous ketones are supplemental ketones — usually beta-hydroxybutyrate (BHB) bound to a mineral salt or an ester — that you drink to raise blood ketone levels from an outside source, independent of diet. "Exogenous" simply means made outside the body, as opposed to the "endogenous" ketones your liver produces during ketosis.

How do exogenous ketones work?

A drink containing BHB salts or a ketone ester raises blood ketone levels within about 30 to 60 minutes, even in someone eating a normal, higher-carb diet.1 The effect is temporary — blood BHB rises for a few hours, then falls back down once the dose is cleared, unlike the steady ketone production your body sustains once it's fat-adapted through carbohydrate restriction.

  • BHB salts pair BHB with sodium, potassium, calcium, or magnesium. They are the form used in most commercial "keto" powders and drink mixes, and they also deliver a dose of electrolytes alongside the ketones.
  • Ketone esters bind BHB to a compound the body cleaves during digestion. Esters raise blood ketones higher than salts but taste harsher and are mostly used in research settings.1

Both differ from MCT oil, which raises ketones indirectly by supplying the liver with fat it converts into ketones, rather than delivering premade ketones directly.

Do exogenous ketones cause fat loss?

No — exogenous ketones do not cause fat loss on their own. Raising blood ketones with a supplement doesn't replicate the metabolic shift of nutritional ketosis, where low insulin drives the body to burn its own stored fat. A body running on exogenous ketones can still be burning glucose for most of its fuel; the elevated BHB reading on a blood ketone meter reflects the supplement, not fat oxidation.1 Early research on appetite and short-term performance is mixed and mostly small-scale, so exogenous ketones are best treated as an adjunct, not a substitute for carbohydrate restriction.

A blood or breath reading can look identical whether ketones came from your liver or from a drink, which is why exogenous ketones can give a false sense of being "in ketosis."
